We offer TeleHealth appointments through HIPPO Health!
Coal Creek Family Medicine is proud to offer our patients Telemedicine as part of our overall approach to trusted and convenient high quality medical care.
What is HIPPO Health?
HIPPO Health is a secure third-party Telemedicine application. It allows our patients to conveniently communicate with our office and physicians from the comfort of their homes.
HIPPO Health allows our patients to have virtual visits with our providers and it allows our patients to send us questions or requests via chat.
Our front office staff is available on HIPPO Health during regular business hours! They can answer any questions you have or process your requests (e.g., make an appointment, submit a medication refill request).
What kind of visits do we offer through TeleHealth?
- Acute issues
- Mental health
- Medication follow-ups
Please note that not all medical services can be offered through Telehealth. Appointment types are at the provider’s discretion, and some medical concerns may require the patient to be seen in person.
How to set up your TeleHealth account:
- Make sure that you are using google chrome to fill out our enrollment form and that you do not have any popup blockers turned on.
- Fill out the Telemedicine Disclosure & Service Enrollment Form.
- Once you submit your payment, you will receive an email from JotForm with a link to sign-up with HIPPO Health. **Make sure to check your spam and junk folders.
- You can use a desktop, tablet, or smartphone to use this service. **Download the app!
What are the subscription fees?
- Individual Monthly Membership
$7.50/month for one person
- Individual Member Yearly Membership
$50/year for one person
- Family Yearly Membership
$100/year for the whole family
Why is there a subscription fee?
We are a small, locally-owned practice, and we strive to keep our care at the highest levels of quality for everyone we see. The reoccurring enrollment fee is designed to maintain excellent staff and adequate staffing levels to support the high-quality care we deliver. This fee also supports our technical platforms and efforts to deliver results of labs, imaging, and testing in the most efficient way possible.
How can I cancel my subscription fee?
Subscription fees are submitted via PayPal. To cancel your subscription, log in to your PayPal account and cancel the reoccurring charge. If you are unable to log in to your PayPal account, or you don’t have an account, you can submit a cancellation request via email to firstname.lastname@example.org. Cancellation requests submitted by email can take up to 7 business days to process.
