Payment Policies
We accept, cash, personal checks, HSA cards, MasterCard, Visa, Discover, and AMEX for your convenience.
We do not take CareCredit
All co-payments are due at the time of service. This arrangement is part of your contract with your insurance company. A $10 fee will be assessed for any co-payment not made at the time of service.
Self-pay patients are those patients without insurance coverage or are receiving a service not covered by their plan. Payment for medical services is due at the time of service.
To ensure correct filing of all medical claims, please have your current medical insurance card or a copy of the front and back with you at each appointment. You can also e-mail us a copy of your insurance card ahead of your appointment to medicalrecords@coalcreekfamilymedicine.com.
