Before you go to the Emergency Room or Urgent Care, please call Coal Creek Family Medicine first. You, as a patient, have 24 hours a day, seven days a week, 365 days a year access to a provider at our practice. If you have an urgent issue outside of regular business hours you may reach our after-hours answering service to be connected to one of our providers by calling our main line.
Making an Appointment
Please call us at (303) 666-7119 to make an appointment. We have several same-day sick appointments available.
Canceling an Appointment
A 24-hours notice is required if you must cancel your scheduled appointment; however, we appreciate you letting us know as soon as possible if you cannot make your scheduled appointment so that we may use the time slot for other patients in need. Cancellations less than 24 hours and No show appointments are subject to a fee.
For your convenience, we are making our patient forms available for you online. Download the forms and print them off to complete them and bring with you for your appointment. This will help to reduce waiting times prior to your appointment!
We e-prescribe! For the most efficient service on routine refills, please call your pharmacy directly.
Certain prescriptions will require authorization from the practice. Please call us at (303) 666-7119 and allow 3 business days to process your refills.
